All applications MUST include: 

1.     Completed School Application form  (Online Google Form – below) 

2.     A copy of the child’s most recent audiogram 

3.     Letter of support from your  Visiting Teacher of the Deaf 

4.     Completed NCSE Form 7 Notification of Enrolment in Special Schools

AND where applicable: 

5.     Completed NCSE Form 2  Application Form for School Transport 

6.     Completed NCSE From 1  Application Form for Special Schools for additional SNA Support
